Schedule: Monday-Friday. Area of coverage is Lane County
Responsibilities- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
- Manage symptom control and provide skilled nursing interventions as needed.
- Educate patients and families on end-of-life care and advance care planning.
- Provide comfort and emotional support to patients and their families during this sensitive time.
- Document patient care activities accurately in compliance with hospice regulations.
- Participate in patient care conferences and team meetings to ensure optimal care delivery.
- Current and valid RN license in the state of practice.
- Strong skills in symptom management and pain relief.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with an interdisciplinary team.
- Compassionate and empathetic approach to patient care.
- Basic life support (BLS) certification required.
- Must have at least 12 months of hospice in home settings experience within the past 3 years.
- HCHB charting experience required.
- PointCare Experience Required
- Weekly pay via direct deposit
- $2,500-$2,700/wk
- Mileage Reimbursement for patient related travel
