We are searching for a talented Nursing Assistant-Cert-Hospice
Hours: 40hrs
Shift: Day Shift, 8:00am - 4:30pm; Every 6th weekend, 2 holidays per year
Location: Visiting Nurse Association - Fairhaven, MA

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Compensation: Pay rate will be determined based on level of experience.
Responsibilities:
Under the general supervision and direction of the RN-Team Lead, Clinical Manager or designee, and using infection control practices, perform patient care activities (ADLs and IADLs) in patients residence (could be SNF, ALF, hospital, group home, home) in accordance with Southcoast VNA policies and procedures and following the Plan of Care developed by the patients case manager, perform light housekeeping, provide respite, assist patients participation in activities program, collect weight and vital signs, assist in meal preparation and feeding, provide vigil support, personal care for the actively dying patient, assist in post-mortem care, communicate pertinent patient information and unusual findings to appropriate clinical personnel, complete all required documentation accurately and in a timely manner and provide exceptional and caring customer service to patient and family.
Qualifications:
- Equal to completion of four years of high school plus specialized courses and training.
- CNA Certification required.
- CPR certification required.
- Three months of related work experience is required.
- Must possess and demonstrate highly effective interpersonal, customer service, and communication skills.
- Completion of initial 8 hr caring for dementia patient training and 4 hours annually required (can be completed as part of orientation).
- Differential applied if currently certified as or upon completion - CHPNA (certified hospice and palliative nursing assistant).
- CPR certification required.
- CNA certification in MA required, additional second certification in RI preferred.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired
Pay Range: USD $16.88 - USD $28.00 /Hr.
Rotation Schedule Requirement : Every 6th weekend, 2 holidays per year
