Are you passionate about making a difference in the lives of youth and families? We are seeking a dedicated Case Manager I - Family Support to join our team, where you'll provide crucial one-on-one support to parents and caregivers in a community setting.

This role involves connecting families with resources, offering coaching, and helping build collaborative relationships with schools, service providers, and other community-based supports.

Responsibilities:

  • Provide family coaching, support, and resources to help connect families with their children’s care team, including schools, counselors, and healthcare providers.
  • Develop and implement care plans, provide advocacy, and assist with service coordination and resource development.
  • Maintain a caseload, ensuring documentation and service delivery are timely and meet best practice standards.
  • Facilitate support groups, maintain the Family Resource Center (FRC) calendar, and engage volunteers for specialized groups.
  • Collaborate with community partners and attend meetings and trainings to continue personal and professional development.

Qualifications:

  • High School Diploma or equivalency certificate required.
  • 1-3 years of related experience.
  • Experience in youth/family case work or wraparound care (preferred).
  • Knowledge of mental health, child welfare, or juvenile justice systems is a plus.
  • Strong communication, organization, and collaboration skills.
  • Valid driver’s license and reliable transportation required.
