Home Care Providers of Texas is currently seeking an LVNCC (LVN Clinical Coordinator) for our Dallas, TX location.

Schedule: Mon-Friday 8-5 p.m.

Licensed Vocational Nurse Clinical Coordinator provides patient care across the entire continuum of care. The Licensed Vocational Nurse Clinical Coordinator for our Skilled Home Health service Functions in collaborative practice with physician(s) and/or nurse practitioner(s) clinical staff, administrative staff.

Job Description:

  • Provides assistance to Regional Manager with supervision of clinical care team staff and acts as a leader of the clinical care team. Maintains a committed and positive attitude, promoting teamwork and harmony by readily assisting team members and customers. Communicates effectively with all patients, families and physicians, as needed.
  • Routinely communicates with and seeks guidance from Regional Manager regarding patient/physician/clinical care team needs and identified issues. Provides report to the Regional Manager as requested.
  • Works with Intake Manager to ensure that all referrals within their service area are processed per agency policy.
  • Works with QA or Intake Dept with procurement of compliant F2F documentation for SOCs.
  • Works with Regional Manager with ensuring that all scheduled patient visits are covered by appropriate personnel. Assists Scheduling Supervisor with management of scheduling issues/conflicts as they arise.
  • Tracks and communicates patient lab/test results to the physician and clinical care team. Ensures these priority documents are scanned into the EMR promptly.
  • Assists QA department with identification of patients requiring specific QA oversight, as requested.
  • May need to assist with the procurement of patient medical supplies/equipment. Works closely with clinical support staff to ensure medical supplies are readily available to clinical staff.
  • Assists the Manager with QA manager review of the visit notes, physician orders.
  • Works with agency leadership personnel and clinical care team staff to schedule/manage/document training/re-training as applicable.
  • Works with field staff on Physician verification process with Physician Updates identified on physician orders, managing via Forcura platform.

Requirements:

  • Graduation from an accredited school of vocational nursing.
  • Minimum 2 (5) years’ experience in physician practice or clinic setting.
  • 3+ years of proven success in leadership roles.
  • Current license to practice vocational nursing in the State of Texas.
  • Demonstrates an understanding of standard clinical procedures, laws. and regulations.
  • Excellent computer skills and familiarity with Microsoft Office programs.
  • Strong organization and time management skills.
  • Superior interpersonal communication skills and the ability to exercise empathy when working with patients and their families.
