Spanning multiple locations, this hospital and health system offers in-patient, home care and hospice in a variety of settings including private homes, nursing facilities, and assisted living facilities.
The Director of Palliative Patient Services helps oversee the day-to-day clinical and operational Hospice activities. This role involves coordinating, supervising, and evaluating patient care services in line with the agency's philosophy, policies, and standards.The Director is responsible for leading the clinical operations of both Hospice and Home Care, guiding the interdisciplinary team, and ensuring high-quality care. They also play a key role in making sure all disciplines work together to provide coordinated, skilled assessments and care for patients.
