Responsibilities:

  • Accurately and timely completes (MCR, quarterly, annual, and change in condition) resident MDS and LTCMI assessments, participates in weekly Care Plan meeting to represent nursing on the Care Plan team.
  • Prepare monthly schedule for Care Plan meetings and distribute schedule to the Care Plan team.
Conducts daily review of documentation process in the medical record and initiate appropriate corrective action as necessary. Complete initial and ongoing care plans.
  • Assist and participate in developing and implementing accurate Care Plans for residents addressing medical condition, skin, bowel and bladder, incontinence program, fall risk, pain risk, psychotropic drug usage, behavior, etc. Reviews and updates quarterly assessments on each resident: AIMS, Braden, Fall Risk, and Physical Restraint, etc.
  • Attends and conducts special studies as needed to identify opportunities to improve system and MDS/LTCMI facilitation.
  • Accurately document written notification regarding denials, transfer notices, and changes in resident status in a timely manner per regulatory practices.
  • Monitors MCR billing system and the MDS RUG levels to ensure timely submissions for payment and correct submission dates.
  • Assists in managing admissions, transfers, and discharge of residents; communicates with residents, family members and management regarding resident status.
Conducts special studies as needed to identify opportunities to improve system and reimbursement.
  • Other duties as assigned.

Qualifications:

  • Education: Graduation from an accredited school of nursing with a current license in good standing as a associate degree nurse by the State of Washington
  • Certification: RAC-CT Certification within 6 months of hire
  • Experience: Six months long term care experience.
  • Ability to comprehend and follow instructions; maintain attention and concentration for necessary periods; synthesize, coordinate, and analyze data, perform simple and repetitive tasks; maintain a work pace appropriate to given work load; perform complex and varied tasks; relate to other people beyond giving and receiving instructions; get along with co-workers and peers; understand the meaning of words and how to use them appropriately and effectively; understand and remember detailed instructions; make independent decisions or exercise judgment based on appropriate information; accept and carry out responsibility for direction, control and planning.
  • MDS/LTCMI training and competency preferred.
