The Clinical Support Services Supervisor, MA may need to perform direct patient care and clinic work within the scope of their licensure as needed.

Compensation: $28.85 - $42.67 per hour, depending on experience.

Responsibilities:
- Provides information and support to the panel specialist, providers, and care teams directed towards the prevention of disease and promotion of health using professional care standards and best practices for quality patient care.
- Ensure management and follow through of team communication, including in basket management and my chart patient communication follow-up.
- Ensures all patient communication notes are documented appropriately in the Electronic Health Record (EHR).
- Works with key stakeholders to improve the efficiency and quality of patient visits.
- Ensure coordination and loop closure with care team and providers to ensure appropriate tracking and follow-up of referrals, and monitor responses related to communication requests.
- Schedule and coordinate scheduling of primary care and specialty appointments as necessary.
- Supports patient navigation by overseeing the tracking of cancer screening diagnostic test results and immunization care gaps, in coordination with provider care plans.
- Facilitates care coordination by ensuring seamless communication between the provider care teams for prescription management and authorizations within the EHR.
- Management of Care Signal remote monitoring program.
- Oversees patient outreach campaigns.
- Ensures adequate staffing to support all aspects of clinical programs and panel management, while also serving as a backup resource when needed.
- Ensures Panel Specialist & CSC staff are appropriately cross-trained, understand department expectations, and up to date with clinical skills.
- Collaborates with Operations (central, clinic and nursing) and Population Health Stakeholders in creating staff trainings.
- Leverages EHR systems to create, manage, and analyze quality data reports and dashboards, identifying patients in need of follow-up for preventive screenings and care gap closures, including cancer screenings, immunizations, and other preventive healthcare measures.
- Work with clinic and practice management staff to troubleshoot and resolve issues using a root cause analysis approach to determine origin of problem and find the best possible solution, and provide recommendations to supervisor.
- Draft and maintain Standards of Procedures (SOP) for existing document processes.
- Ensure productivity by tracking against set key performance indicators.
- Facilitates audits and ensures timely and accurate correction of any identified discrepancies.
- Maintain accurate employee time records by collecting, reviewing, and approving timesheets to ensure compliance with company policies and payroll accuracy.
- In conjunction with the Human Resources (HR), manage the recruitment and performance of staff.
- Partners with Human Resources (HR) to manage staff recruitment, performance, and development, providing direct oversight of staff through evaluations, coaching, and corrective action plans.
- Performs additional duties, as assigned by Director of Central Operations and in support of quality assurance and improvement.
- Performs all duties and services in full compliance with TVHCs Service Excellence Standards.
- Current, valid California Medical Assistant (MA) Certification from an accredited institution required.
- Minimum of three (3) years of MA experience required.
- Current, valid CPR certification.
- Leadership experience preferred, including the ability to mentor staff, coordinate team efforts, and contribute to process improvements.
Qualifications:
- Bilingual English/Spanish preferred, but not required.
- Strong project management skills, including the ability to plan, execute, and track progress effectively.
- Proficiency in electronic health records (EHR) with the ability to support and train others in its use.
- Excellent interpersonal and communication skills to build trust and engage effectively with diverse patient populations and staff.
- Demonstrated leadership abilities, including staff motivation, team building, and taking initiative in clinical and programmatic areas.
- Demonstrates flexibility in regard to job duties and assignments.
- Have ability to travel to various locations as needed.
