Department: Care Coordination and Social Work

Job Summary:

This is a 12-week assignment requiring a Monday to Friday schedule. The Licensed Master Social Worker will provide comprehensive social work services in an inpatient hospital setting, managing a caseload of 25 to 30 patients.

Responsibilities:

• Conduct in-depth psychosocial assessments to evaluate patients' needs and challenges.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Provide support and resources for patients dealing with complex issues such as hospice care, hemodialysis, substance abuse, guardianship, and ethical cases.
• Maintain accurate documentation in accordance with hospital policies and procedures.
• Utilize EPIC and Allscripts systems for patient management and record-keeping.
• Advocate for patients and their families, ensuring access to necessary services and support.

Qualifications:

• Master’s Degree in Social Work (MSW) (required).
• Licensed Master Social Worker (LMSW) by the State of New York (required).
• Inpatient hospital experience (required).
• Experience with EPIC and Allscripts software (preferred).
• Strong psychosocial assessment skills and the ability to manage complex caseloads (required).
