JOB SUMMARY:
Responsible for the provision of safe, effective, and comprehensive medication management services. Deliver optimal medication therapy to patients with a broad range of disease states. Provide direct patient-centered care and integrated pharmacy operational services in medical offices practice via collaborative practice agreements.Participate in medication management while providing comprehensive and individualized pharmaceutical care to patients assigned. Provide pharmaceutical care services including assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent adverse medication events.
Support in optimizing performance in value-based programs. Serve as departmental resource and liaison to other departments, clinical operations, decision support, and retail pharmacy operations. Provide medication and practice-related education/training to medical staff and residents.
Responsible for dispensing prescriptions in retail pharmacy setting by following standard operating procedures for receiving, filling, dispensing, logging, and maintaining quality assurance of all prescriptions dispensed.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Delivers the HSO brand promise of putting the patient/customer at the center of all we do.
- Oversees prescription processing activities in pharmacy retail setting to ensure quality and standard of practice is maintained
- Through Collaborate Practice Agreements:
- Develop clinical pharmacy programs according to policies and regulations
- Provide services for the management and treatment for Diabetes Mellitus, Hypertension, Hyperlipidemia, Asthma, COPD, Hepatitis C, HIV PrEP, and Smoking Cessation.
- Reviews records of patients to determine the appropriateness of medication therapy
- Evaluates patient’s condition to ensure all issues are being treated
- Develop effective medication plans that minimize the risk of adverse side-effects
- Consults on dosages, medication substances etc.
- Advises on the correct administration of drugs
- Assesses the results of pharmaceutical treatments
- Collaborate with healthcare professionals to ensure optimal patient care
- Keeps accurate documentation of medication plans and patient progress
- Has strong leadership skills and ability to work closely with physicians and staff
- Comply with all Federal and State pharmacy laws
- Comply with all pharmacy procedures, rules, and regulations
- Exhibits brand characteristics of Friendly, Accessible, and Credible.
- Other duties as assigned
