Job Overview:

We are seeking a dedicated and detail-oriented Pharmacist to join our inpatient team. The ideal candidate will be responsible for reviewing medication orders, dispensing prescriptions, ensuring patient safety, and collaborating with healthcare professionals to optimize drug therapy.

Key Responsibilities:

• Medication Order Review: Verify accuracy, dosage, and appropriateness of medication orders.
• Dispensing Medications: Prepare and dispense prescribed medications with proper labeling and quality standards.
• Patient Safety: Monitor for drug interactions, allergies, and contraindications to prevent adverse reactions.
• Clinical Consultation: Work closely with physicians and nurses to provide medication therapy recommendations.
• Medication Therapy Management: Assess and counsel patients on medication regimens for optimal outcomes.
• Education & Training: Educate healthcare staff and patients on medication use, side effects, and safety.
• Compounding Medications: Prepare sterile and non-sterile compounded medications as needed.
• Pharmacovigilance: Monitor and report adverse drug reactions or medication errors.
• Inventory Management: Maintain proper medication storage, track stock levels, and prevent shortages.
• Patient Discharge Counseling: Educate patients on new or adjusted medications upon discharge.

Requirements:

• Minimum 1 year of pharmacist experience
• Valid NYSED Pharmacist License
• Basic Life Support (BLS) Certification
• COVID-19 Vaccination Card
