Join UChicago Medicine as a Home Infusion Registered Nurse

The Home Infusion Nurse provides intravenous medication administration for patients in the home and the infusion suite setting. The professional nurse plans, coordinates, and performs home infusion services with emphasis on community health education/experience.

The professional nurse works cooperatively with pharmacy staff to establish priorities and goals of patient care.

Essential Job Functions
  • Administers medications and treatments as prescribed by the physician.
  • Monitors patient response to interventions.
  • When providing home infusion skilled nursing services, will assess and monitor patient for anaphylaxis and intervene as needed.
  • Successfully instructs patient on home infusion therapy self- administration, provide direct nursing care in patient’s home for the administration of home infusion therapies, and troubleshoot patient’s infusion concerns.
  • Regularly re-evaluates patient nursing needs.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physicians orders as required.
  • Participates in on-call duties as defined by the on-call policy.
  • Completes an initial assessment of patient and family to determine home care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Develops a care plan which establishes goals, and incorporates therapeutic, preventive and nursing actions, as well psychosocial support as needed. Includes the patient and the family in the planning process.
  • Complete all required patient care documentation, nursing plan of care and reports.
Position Qualifications
  • One to two years of acute care experience in an institutional setting.
  • One year of home health care experience preferred.
  • Oncology experience preferred.
  • Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits.
  • Infusion experience preferred.
  • Associate Degree in Nursing; BSN degree or higher preferred.
  • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
  • Current state of Illinois nursing license.
  • ACLS within 6 months of hire.
  • Chemotherapy Biotherapy Safe Handling course required within 1 year of hire.
  • ONS Chemotherapy/ Immunotherapy Certificate within 6 months of hire.
