Description:
We're seeking a dedicated PRN Social Work Case Manager to work with our Social Work Case Management team at Emory University Hospital. As part of our team you will be responsible for coordinating clinically complex patient care needs across the health care continuum.
You will assess patients for transition needs including identifying patients at risk for readmission. The Social Worker / Case Manager will appropriate the facilitation of high quality clinical and cost outcomes; negotiate, procure and coordinate services and resources needed by the patients in a timely and effective manner.
You will collaborate with Utilization Review in order to identify and address barriers to timely and effecient care delivery and reimbursement.
MINIMUM QUALIFICATIONS- Masters in Social Work from an accredited Institution.
- At least 1 year experience preferred, experience in an Acute Care setting preferred.
- Positions in this classification require MSW license or license eligibility in the State of Georgia.
- Knowledge of software applications.
- ACM, CCM preferred.
- For Behavioral Health job locations, Master's degree in Psychology, Counseling, or related field from an accredited institution will be accepted in lieu of Master's degree in Social Work and license eligibility for LAPC/LPC in the state of Georgia will be accepted in lieu of license eligibility for LMSW to meet the minimum job requirement.
