We have an exciting opportunity for a full-time, Social Worker to join Heritage Manor. The Social worker will develop a social work care plan including goals based on the assessment of needs and completes psychosocial assessments. Participates in weekly interdisciplinary care planning team meetings: develops comprehensive treatment plans/discharge plans, when appropriate.

Initiates participation of resident/family members in care planning process. Provides explanation to families and residents of Medicare and Medicaid, refers families and residents to appropriate resources. Demonstrates an understanding of, and embraces, the mission statement of the Christian Health.

Education:

  • Bachelor's degree in social work, psychology, sociology or Master's degree in social work related field required.
  • Certified Social Worker (CSW) or Licensed Social Worker (LSW) required.

Schedule: Tuesday - Saturday 9:00am - 5:00pm

QUALIFICATIONS/REQUIREMENTS:

Qualifications:

  • Bachelor's degree in social work, psychology, sociology or Master's degree in social work related field required.
  • Certified Social Worker (CSW) or Licensed Social Worker (LSW) required.
  • One year long term care social work experience strongly preferred.
