In this inspiring role, you hold the power to perform patient comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.
Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a harmonious team of caregivers.

Qualifications:
Active RN License in state of service
One (1+) Year of Clinical Experience required; additional home health experience preferred
Must obtain and maintain CPR Certification
