Overview:

The Admissions Supervisor supports the Admissions Department to ensure timely admissions with a high degree of quality and proactive customer service. Facilitates improved access to hospice services through education and role modeling. Applies expert clinical judgment to ensure optimum quality of patient care and services, while complying with governmental and agency regulations.

Supports agencies standards and protocols through education and auditing.

The Elizabeth Hospice is San Diego’s oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life’s journey and for those who grieve.
Salary: $90,000 - $120,000 annually. Subject to experience and qualifications.
On-call: Ocassional rotation
Work Schedule: Monday - Friday, 9am - 6pm
Work Location: Escondido, CA

Travel: Occassional travel is required throughout San Diego and South Riverside counties.

Responsibilities:

  1. Provides direct communication to all patients and families referred to hospice or palliative care when referral is received to deliver customer service and support with emphasis on patient/family needs and concerns.
  2. Acts as a clinical navigator for patients, family members and referral sources. Identifies needs and communicates effectively to the Access Team and Admissions Director to ensure good customer service and emotional support.
  3. Supports the Admission process while working with internal teams/departments to enhance referral-admit conversion to 70% or greater through great customer service and support.
  4. Follows protocols to ensure compliance with all federal, state, and local health care regulations as they affect the provision of admissions services.
  5. Demonstrates leadership in referral and assessment decisions and communicates effectively with both external and internal customers,
  6. Monitors utilization of personnel to ensure acceptable/safe productivity levels in conjunction with the Director and Managers.
  7. Helps to maintain agency census goals while nurturing relationships within community referral base and collaborating with Director of Sales & Marketing, and Chief Medical Officer
  8. Collaborates with the Admissions Director to resolve quality concerns.
  9. Reviews Admission notes to ensure adequate medical eligibility is documented and other required documentation is included in the record Seeks necessary pre-approvals per agency policies.
  10. Provides clinical support and coordination for referral sources including coordinating admissions from out of the area, as needed.
  11. Ensures agency standards are followed for all non-admissions.
  12. Compiles required reports such as staffing schedules, appointments, changes, and other items as needed for the Admissions department and provides follow up in a timely manner with staff to ensure compliance. Assists with projects as assigned.
  13. Assists with Admissions RN duties such as discharge planning, medication ordering, transport, etc.
  14. Provides phone call support for clinical questions from families or referral partners.
  15. Keeps current with all department procedures and policies.
  16. Provides staff support including various clinical approvals and verifications, as well as maintaining necessary electronic and hard copy resources with Admission Director’s approval.
  17. In collaboration with Admissions Leadership manages daily schedule. Posts assignments to field staff in a timely manner.
  18. In the absence of the Admissions Director, directly supervises all clinical staff up to and including daily assignments, active admission support and disciplinary actions if needed.
  19. Participates in the on-call clinical management rotation.
  20. Other duties as needed and requested.

Qualifications:

  1. Must have an Associate’s Degree, Bachelor’s Degree preferred, and at least 4 years’ experience with 1-2 years preferred in acute care, or equivalent combination of education and experience, and at least 2 years hospice experience required.
  2. Current CA RN licensure required.
  3. Current CPR certification and negative TB skin test.
  4. Prefer certification as a hospice palliative care nurse (CHPN).
  5. Computer skills at minimum of Intermediate level
  6. Current California driver’s license and proof of auto insurance required.
  7. Bilingual/Bicultural Spanish/English preferred.
