Lead Outreach Coordinator

CCHRC is seeking a highly motivated and positive individual to fill our Lead Outreach Coordinator position.

Lead Outreach Coordinator will be responsible for marketing and implementing an admission outreach plan to raise awareness and interest about our facility within the community. They will also communicate a positive image of our facility and project the facilities strengths to potential residents and family members.

• This position requires strong communication skills, both written and oral.
• The ability to speak in front of large and small audiences.
• Travel away from the facility to other health facilities, potential resident homes and businesses to promote a positive image of our facility and gain potential referrals.
• To develop a promotional strategy to inform and remind potential residents of the benefits of our facility.
• Develop a relationship with discharge planners, doctors and hospital administration staff to communicate the quality of care they can expect at our facility.
• Determine the criteria for potential residents and develop an admissions action plan.
• The Outreach Coordinator will also work closely with the Admissions Nurse and the Admissions Social Worker to insure the admission process flows smoothly for the residents, family members and facility staff.
• The Admissions Team and or Administrator may also require other duties be performed by the Lead Outreach Coordinator.
• Full time position, M - F 8:00a - 4:30p
• Must have flexible availability
• Pay rate based on experience
