Overview:
We're looking for a home health aide to join our team! The home health aide is an employee trained to provide designated personal care services to enable the patient to achieve and/or maintain optimal physical and emotional wellbeing.
Under supervision and in accordance with Agency policies provides care following. written instructions prepared by a registered nurse. The Therapist will prepare written instructions when the Aide is to provide services which are an extension of the therapy program.Successfully competes additional training before performing duties such as changing or applying non-sterile dressings or applying prescribed ointments and topical medicines, gastrostomy tube feedings, and eating for person having special needs because of AlDS, Alzheimer's, mental retardation or terminal illness.
Responsibilities:
- Provides personal care including oral hygiene, skin, hair and nail care, and shaving male
- Assists with simple exercise programs; transfers, ambulation, and activity as directed by the Home Health Nurse or Therapist.
- Assists patient with medications which are ordinarily self-administered.
- Assists patient with use of toilet facilities and durable equipment, bedpan, and commode, 6. Takes and records blood pressure, temperature and pulse and respiration.
- Assists patient to achieve maximum self-reliance through relearning and modifying activities of daily living.
- Plans, shops for, and prepares nutritious meals, including special diets when required.
- Performs household tasks essential to maintain patient in clean safe environment (changing bed linens, dusting patient's room, patient's laundry, etc).
- Promotes mental alertness of patient through conversation and purposeful activity.
- Measures and records fluid intake and output,
- Promptly reports changes in the patient's condition, behavior or appearance to the Home
- Completes appropriate records on a daily basis in accordance with Agency policy and procedures.
- Attends staff meetings and in-service programs as scheduled.
Qualifications:
- A certificate of completion from an approved home health aide training program; or,
- Certified as a nurse aide by the State of Pennsylvania; or
- has completed first year clinical studies at an N.L.N. approved school of nursing or
- High school diploma or GED preferred,
- One (1) year experience as nurse aide in a hospital, nursing home, or home health agency.
- Successfully compietes Agency competency test.
- Valid driver's license.
- Ability to follow oral and written instructions.
- Ability to complete simple reports
- Ability to maintain confidentiality of patient information.
- Ability to demonstrate fact and good judgement inworking with those who are ill and their family/significant others
- Ability to use good body mechanics to meet physical job demands.
