Responsibilities:
Coordinates the referral intake process under the direction of the Clinical Supervisor/Manager/Director. Records and maintains timely and accurate referral data. Provides clinicians with required information needed to start services for a new patient, and contacts physician for any concerns.
Qualifications:
EDUCATION, EXPERIENCE, TRAINING- Previous home care experience preferred.
- Graduate from an accredited school of nursing.
- Current license as a registered nurse.
- Familiar with state and regulatory requirements.
- Strong verbal and written communication skills. Strong interpersonal skills. Strong problem solving skills. Strong time management skills. Ability to work independently.
A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $XX.XX to $XX.XX.
