Job Description:

  • The Licensed Social Worker is responsible for providing social work services to patients, families, and caregivers to ensure emotional and psychosocial support during medical care. The Social Worker collaborates with the healthcare team to address issues related to mental health, discharge planning, and community resources, contributing to the holistic care of the patient.

Responsibilities:

  • Conduct comprehensive assessments of patients’ psychosocial needs, including emotional, financial, and social factors impacting health and wellness.
  • Collaborate with the interdisciplinary healthcare team, including physicians, nurses, and case managers, to develop individualized care plans that address both medical and social needs.
  • Assist in planning and coordinating patient discharge, including identifying appropriate community resources, arranging follow-up care, and ensuring continuity of care.
  • Provide immediate support and counseling in crisis situations such as trauma, terminal illness, or family emergencies. Help patients and families navigate difficult decisions and transitions.
  • Offer individual and family counseling to address issues such as coping with illness, grief, substance abuse, mental health challenges, and adjustment to healthcare settings.
  • Identify and connect patients and families to available community resources such as financial assistance, housing, mental health services, and home healthcare.
  • Act as a patient advocate, ensuring that patients’ rights are respected, and helping them navigate complex healthcare systems. Work to reduce health disparities by addressing barriers to care.
  • Maintain accurate and thorough records of assessments, care plans, interventions, and outcomes in the electronic medical record system in compliance with hospital policies and regulatory requirements.
  • Provide education to patients, families, and staff about social work services, available resources, and coping strategies. Offer guidance on healthcare policies, insurance, and advance directives.
  • Work closely with hospital departments, community organizations, and external agencies to coordinate services that meet the patient’s needs. Communicate effectively with all members of the healthcare team.

Required Minimum Education:

  • Bachelor’s Degree in Social Work (BSW) from an accredited institution required.
  • Master’s Degree in Social Work (MSW) preferred.
  • Required Minimum Licensure/Certification:
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of Georgia required.
  • Basic Life Support (BLS) certification may be required depending on the role.

Required Minimum Experience:

  • A minimum of 1-2 years of experience in a healthcare setting, such as hospitals, nursing homes, or rehabilitation facilities, is preferred.
  • Experience in discharge planning, crisis intervention, and working with interdisciplinary teams is advantageous.
