Monday and Thursday, 7-am to 11-pm
This On-Call Advanced Practice Clinician (Mid Level Provider - Nurse Practitioner or Physician Assistant) at NE 122nd and Glisan, Portland.
The position will be responsible to provide patient-centered care in the clinical setting, including: performs sexual health screening, genital and gynecological exams, and diagnosis and treatment of genital and sexually transmitted infections (STI); provides evaluation and treatment of wound and skin conditions, including incision and drainage, and management of acute and chronic wounds; performs venipuncture as needed, orders and interprets laboratory studies; prescribes medications such as contraception, HIV Pre-Exposure Prophylaxis (PrEP), doxy-PEP, and antibiotics; provides follow-up patient consultations, telehealth visits, and other medical conditions as per clinic guidelines and patient needs; educates patients on goal-based, appropriate preventative care, chronic care, or urgent care needs, as well as the use of prescribed medications and treatment; develops and directs patient-centered care plans with the assistance of other disciplines within the care team.Patient treatment may include developing and carrying out complete patient treatment plans when necessary.
This position will work in the Menlo Park Plaza Building.
As the Advanced Practice Clinician
, you will:
Work cooperatively with the team to provide effective and efficient evaluations, diagnosis, treatment plans and prevention services
Consistently demonstrates effective, positive and respectful communication with patients and staff including: active listening, empathy, courtesy, cultural sensitivity, and the ability to promote modification of health behaviors with clients
Perform sexual health history, physical examination and specimen collection as outlined in Program clinical guidelines
Formulates appropriate diagnoses and implements treatment plans based on clinical practice guidelines, policies and community standards of practice
Provide health teaching to patients based on clinical practice guidelines, other designated guidelines, and community standards of practice
Participate in precepting students as part of Region X training site and other students/observers (medical, nursing, pharmacy, etc.)
Participate in clinical research studies, including consenting, data collection, and other activities
Provide daily direction to Community Health Workers to implement aspects of care plans within their scope and training
Provide appropriate advice on the management of STIs to county or community health care providers, following Harm Reduction Clinic and STI Program clinical guidelines and in accordance with Centers for Disease Control and Prevention (CDC) treatment guidelines
Services: syringe exchange, sexual health services, overdose rescue kits (naloxone and fentanyl test strips)
Requirements:
Education/Training:Training and experience for this classification is Board Certification and State licensure as a Nurse Practitioner or Physician Assistant and
Experience: One (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position
Other:
CPR/BLS certification issued by the American Heart Association or like organization
Nurse Practitioner, Registered Nurse, or Physician Assistant License issued by the Oregon State Board of Nursing or Oregon Medical Board

One year of community health nursing or primary medical care experience with wound and skin care
