Department: Skilled Nursing Facility

Job Responsibilities:

• Monitor patient health conditions, including vital signs, and report any changes.
• Administer prescribed medications and treatments as directed.
• Provide basic patient care, including wound dressing, enemas, and dressing application.
• Observe patients and report any adverse reactions to medications or treatments.
• Collect samples for testing, perform routine lab tests, and document results.
• Assist with patient hygiene, feeding, and daily living activities.
• Offer emotional support to patients and their families.
• Maintain accurate patient health records and report updates to registered nurses and physicians.
• Participate in ongoing education and training to enhance nursing skills.

Qualifications:

• Previous experience in long-term care or rehabilitation nursing preferred.
• Strong communication and interpersonal skills for patient and team interactions.
• Ability to work independently and collaboratively in a fast-paced healthcare environment.

Requirements:

• Active and valid Licensed Practical Nurse (LPN) license in New Jersey.
• CPR/BLS certification required (Advanced certifications preferred).
• COVID-19 vaccination card.
