Licensed Vocational Nurse / Licensed Practical Nurse - LVNLPN24-14165

Duration: 13 weeks

Responsibilities:

• Continuously observe residents for changes in mental and physical health.
• Notify the supervisor of significant findings and discuss the need for physician involvement.
• Provide a comprehensive Resident Status Report during shift changes, including updates on each resident’s status and events from the shift.
• Administer medications and treatments as prescribed by physicians.
• Document all medications and treatments accurately using a computerized system.
• Perform wound care, including the use of wound vacs.
• Verify daily appointments (Physical, Occupational, and Speech Therapy).
• Ensure the activity board is up-to-date.
• Remind staff to complete tasks such as ambulation, weight checks, blood pressure readings, and documenting nutritional charts.
• Monitor Licensed Nursing Assistant (LNA) documentation, updating the LNA clipboard as needed.
• Oversee infection control practices and ensure the unit is clean and hazard-free.
• Maintain ongoing communication with the Nurse Manager/Nursing Supervisor regarding resident issues.
• Work closely with LNAs to document and report relevant observations.
• Differentiate between acute and non-emergency issues and escalate when necessary.
• Communicate with residents’ families to resolve concerns, notify them of new physician orders, and obtain consent when needed.
• Handle admissions, discharges, and transfers of residents, ensuring proper documentation and communication.
• Complete 24-hour reports for assigned units, check new orders, and track residents on antibiotics or precautions.

Requirements:

Experience:

• Minimum of 6 months in Long Term Care (LTC) - REQUIRED

Certification:

• Basic Life Support (BLS) - REQUIRED

Skills Checklist:
