Case Manager - Individualized Services
PathWays is celebrating its 50th anniversary of serving individuals with developmental disabilities and their families in Sullivan and Lower Grafton counties of NH in 2023 and we need people to continue our services for another fifty years.
Would you like a position that allows you to play a role in assisting people in living their best life? Do you enjoy problem-solving and collaborating with individuals and families? If so, join us as a Case Manager/Support Broker in our Individualized Services group and support individuals and families who want more flexibility and control of the services received. In this role, you will carry a caseload of 25 to 30 individuals and after one year you will know you were successful if:
- You possess a solid understanding of the Individual Service Agreement process;
- You have established a fluid schedule of visits and team meetings with your individuals and families; and,
- You have a comprehensive understanding of the needs and circumstances of the individuals and families you serve.
Key to being successful in the role are excellent time management skills, the ability to frequently and efficiently switch gears between tasks, and strong interpersonal skills.
A Bachelor’s degree in a related field is highly preferred and 2 to 3 years minimum of case management experience. Extensive case management experience may substitute for a degree. You need to have a minimum of 2 years of human services experience and we prefer experience with developmental disabilities as well.Excellent verbal, interpersonal, and written communication skills are key. Intermediate proficiency in MS Office (Word, Excel, PowerPoint) as well as an ability/willingness to learn new programs is essential. You will travel throughout the region for meetings with families so a valid driver's license and auto insurance are required.
Mileage is reimbursed.
