Responsibilities:

The Occupational Therapist develops and implements the home care plan for the occupational rehabilitation of patients under the direction of the attending physician and the appropriate clinical leader.- Performs initial occupational therapy patient evaluation.
- Establishes and performs occupational therapy treatments according to the physician’s plan of treatment and in compliance with state and federal guidelines.
- Provides patient care education to patients and other caregivers.
- Participates in case conferences, summary reports, quarterly record review and annual program review, in-services and special case orientations.
- Maintains productivity standards and availability requirements.
Qualifications:
- Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association.
- Active registered OT license in the state of Arizona
- One year of OT experience required
- Passion for excellence in quality of care
- In Washington State, all providers must meet NPI requirement and furnish proof prior to onboarding
