Position Summary:

Clinical Pharmacy Specialist, Transitions of Care

Orlando Regional Medical Center, an 808-bed tertiary care teaching facility, seeks a highly qualified clinical pharmacist to participate in direct patient care activities in Internal Medicine and to support the delivery and development of pharmacotherapy programs.

Orlando Regional Medical Center is the flagship hospital of Orlando Health, one of Florida’s most comprehensive not-for-profit Health Systems that includes 10 wholly owned hospitals.
  • Central Florida’s only Level I Trauma Center with over 4,000 trauma patients per year
  • More than 350 Medical Residents and Fellows and 10 Pharmacy Residents (PGY-1 Pharmacy Residents (4), PGY-2 Critical Care Pharmacy Residents (2), PGY-2 Emergency Medicine Pharmacy Resident (1) , PGY-2 Pediatric Pharmacy Residents (2), PGY-2 Oncology Pharmacy Resident (1)
  • 115 Team Member Pharmacy Department providing service 24 hours/ 7 days per week
  • A team of 17 PGY-2 or equivalent trained Clinical Pharmacy Specialists focusing on Internal Medicine, Medical Critical Care, Surgical Critical Care, Neurocritical Care, Surgery/Trauma, Nutrition Support, Emergency Medicine, Hematology/Oncology, and Infectious Diseases.
  • Majority of Clinical Staff Pharmacists are PGY-1 Residency trained
  • Decentralized and Satellite services specializing in Critical Care, Burn/Trauma, Intermediate Critical Care, Cardiology, Orthopedics, Surgery, Medicine, Oncology, and the Operating Room.
  • CPOE / EPIC, Alaris Smart IV Pumps, SwissLog BoxPicker, Omnicell
  • Available resources: Drug Information Center, Medical library with librarian support, Drug information databases, Pharmacy Intranet Resources.
  • Extensive committee involvement, including but not limited to, Pharmacotherapy, Medication Use Subcommittee, Bioethics, Geriatric Quality Improvement, Antimicrobial Stewardship Team, Pain Management Subcommittee, Diabetes Safe Team, and Heart Failure Taskforce.

Position Overview

This position functions as part of a new transitions of care team comprised of pharmacists and technicians. The position offers an excellent opportunity to directly impact patient care, medication safety, and decrease readmissions.

Responsibilities:

Essential Functions
  • Under the general direction of the Pharmacy Clinical Manager, is responsible for development, implementation, and maintenance of clinical pharmacy services at the designated practice site.
  • Demonstrates the ability to plan, organize, execute, and monitor clinical pharmacy services designed to improve the quality of patient care while effectively utilizing resources.
  • Facilitates coordination of activities and responsibilities of the pharmacy department to ensure that clinical services are provided with sufficient scope and coverage to meet the requirements/needs of the patient population and community served.
  • Organizes and maintains appropriate documentation of clinical activities for purposes of pharmacy management and hospital accreditation.
  • Conducts a clinical practice, such as facilitating verbal and written pharmacotherapy and pharmacokinetic consultations with physicians and other healthcare providers.
  • Assists, as applicable, with nutritional support consultations with physicians, dieticians, and other healthcare providers.
  • Continuous professional development related to clinical practice.
  • Supports staff pharmacist clinical education and training associated with core competencies.
  • Develops policy and procedures, guidelines, and programs encompassing clinical pharmacy services as necessary.
  • Participates on multidisciplinary committees, such as the Pharmacotherapy Committee and subcommittees.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.
Other Related Functions
  • Participates in pharmacy and medical resident training through multidisciplinary patient care rounds, and formal educational
programs (site specific).
  • Serves as preceptor to students and interns for affiliated colleges of pharmacy (site specific).
  • Performs other duties as assigned.

Qualifications:

Desired/Required Credentials

Doctor of Pharmacy degree combined with PGY-1 residency. PGY-2 residency experience in internal medicine, pharmacotherapy, or equivalent experience preferred. Florida pharmacy licensure eligibility required.

Experience
Must meet one (1) of the following:
 Two (2) years completion of an ASHP-accredited residency or Fellowship in specialty area consistent with practice needs of the institution.
 One (1) year completion of an ASHP-accredited residency plus specialty practice experience (1+ years) consistent with the practice needs of the institution.

 Five (5) years of clinical pharmacy experience consistent with the practice needs of the institution.
