Responsible for ensuring high quality experience for Luminary Circle members by providing assistance, navigation, and support through the healthcare system. Partners with members, program Director, Coordinator and liaisons, and the healthcare team to identify barriers and solutions while exhibiting a high degree of compassion, professionalism, and customer service skills.

Ideal Candidate:

High School Diploma required, Bachelors degree preferred. Completion of minimum 5 years relevant work experience in patient care setting.

Qualifications:

  • Completion of minimum 5 years relevant work experience in patient care setting.
  • Requires strong conflict management skills for effective member assistance and demonstrated ability to sustain quality standards.
  • Knowledge of EPIC preferred.

Education Essential: High School Diploma or GED

Responsibilities:

  • Serves as liaison between members and organization
  • Creates collaborative relationships with members and clinical staff across a variety of areas within PHS, PHP, and PMG and externally to ensure members receive quality healthcare services
  • Respond when requested to deescalate raised concerns of members while objectively partnering with care team to identify solutions
  • Perform daily proactive rounds with admitted members in multiple care settings establishing a personal connection through effective communication
  • Work collaboratively with multidisciplinary teams and liaisons, communicate professionally, work independently
  • Consistently follows policies, procedures and approved practices
Maintains member confidentiality of personal, medical, and financial information
  • Uses information technology and various electronic systems to assess, document, and communicate patient care and work
