REQUIREMENTS: Current Resume
LPN- PA license (in good standing)Sub-Acute/SNF/Long-Term Care experience (1+ year)
Current - BLS/CPR
- Current references (RN- managers)
- Step PPD (within the last 12 months)
Vaccination record: MMR, Varicella, Hep B
Must be able to pass criminal clearance check/ back ground checkMust be able to pass urine drug screen
