Responsibilities:
LPN Job Summary
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
LPN Job Functions- Provides direct patient care within the established Plan of Care and as defined in the State Nurse Practice Act.
- Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers. Assists team members as needed to ensure that this level of service is consistently provided.
- Implements planed interventions to meet the physical, psychological, social and environmental needs of the patient.
- Evaluates the patient’s responses to the approved plan of treatment.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of hospice services, appropriately reporting any identified needs to the interdisciplinary group.
- Participates in Continuous Care as requested.
- Participates in clinical on-call program.
- Attends Interdisciplinary Group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement.
- Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
- Performs other duties as assigned by the registered nurse.

Qualifications:
LPN Position Qualifications- Graduate of an accredited practical nurse or vocational nursing program
- Current license to practice in the state of practice
- Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience
- Current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Demonstrates excellent observation, problem solving, verbal and written communications; skills per competency checklist
- Shows ability to organize and prioritize workload Independently
- Works indoors in patient homes
In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
