Responsibilities:
What You'll Do:
The Case Manager - Adult and Youth is responsible for managing the care of individuals, including both youth (adolescents) and adults, who are in crisis and stabilizing from acute episodes. This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans.The Case Manager works closely with an interdisciplinary treatment team to coordinate care and discharge planning for both youth and adult individuals, their families, schools (for youth), local providers, and other referral sources. This position also carries a caseload and serves as an ambassador to community partners.
- Admission & Consent
- Facilitate admission intakes, educating and informing individuals and their families about consents for treatment and authorizations for use and disclosures.
- Treatment Planning:
- Collaborate with the Interdisciplinary Treatment Team to develop and implement individualized, person-centered Treatment Plans appropriate for brief intervention.
- Complete and review signed Treatment Plans with each individual and their families (where applicable) to ensure understanding and agreement.
- Collateral Gathering & Assessment:
- Gather collateral information from individuals, family members, schools (for youth), and other involved parties to conduct a thorough assessment of needs for treatment and discharge planning.
- Care Coordination:
- Actively coordinate care and provide referrals to ensure services are aligned with both facility staff and external providers.
- Identify and address gaps in service needs, making appropriate recommendations to meet the individual’s needs upon discharge.
- Discharge Planning:
- Collaborate with the interdisciplinary treatment team to engage with, monitor, and communicate with individuals and their families, ensuring safety and meeting their needs during the discharge process.
- Documentation:
- Maintain accurate clinic records, documenting all activities related to service delivery, including brief interventions, coordination of care, discharge planning, and treatment planning.
- Obtain necessary signatures from individuals or their guardians regarding treatment and other required documents.
- Community & System Knowledge:
- Maintain a working knowledge of the Pennsylvania State behavioral health system, including potential funding sources, 302 involuntary legal processes, and resources specific to both youth and adults.
- Professional Development:
- Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.
- Demonstrate competency through post-testing, skill observation, and performance assessments as conducted by direct supervisors.
- Performs all other duties as assigned
Qualifications:
What You'll Bring:
- A Bachelor’s degree in behavioral health or related field and 2 (two) years of related experience
- Must pass the following background checks
- Child Abuse Clearance
- FBI Criminal Background Check
- Pennsylvania State Police Criminal Record Check
- Level 1 Fingerprint clearance card4 years of behavioral health experience
- Knowledge of the Pennsylvania State behavioral health system
- The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
It would be great if you had:
- Master’s degree in behavioral health or related field
- Licensed Mental Health Professional (LMHP)
- Experience working with children, youth, families, and adults with behavioral health issues
- Knowledge of and experience with the Pennsylvania involuntary treatment process
- Ability to perform Safe Clinch restraint techniques

The standard hiring range for this position in Harrisburg, PA is $25.00 - $27.00 per hour. The exact compensation will be based on factors such as years of experience, competency, and skill level.
