Supportive Housing Case Manager
Overview:
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Supportive Housing Case Manager will provide case management services to residents of our brand-new Supportive Housing program, Confluence Court, in Binghamton, NY.The Case Manager will provide services for up to 30 residents receiving ongoing care for Substance Use Disorder with the support of a team of their Team Leader and Peer Specialists.
The program's planned opening is August 2025, but we are looking to hire a Team Leader immediately to begin the process of promoting the program and accepting referrals. The position initially starts via remote work but will transition to an on-site role upon the opening of the program.
Pay Rate: $25.00 - $29.00 per hour

Responsibilities:
- Maintains a case load and performs primary case work responsibilities including:
- Screening, intake, orientation
- Referrals
- Counseling
- Crisis intervention
- Patient/resident education
- Case management
- Reporting and record keeping
- Consultation with other professionals
- Coordination of care with other community providers
- Discharge planning
- Trains of staff, volunteers, and students/interns
- Performs Quality Assurance activities
- Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
- Monitors provision of services when assigned.
- Evaluates programs and identifies strengths/weaknesses.
- Assists in problem resolution and implementation of supervisor approved program modifications.
- Participates in program administrative and clinical committees as assigned.
- Represents agency to the community.
- Other duties as assigned by supervisor.
Qualifications:
- High School Diploma or GED required, Associate's degree in a related human services field preferred.
- One year of experience working with behavioral health and/or substance abuse issues preferred.
- Valid NYS Driver’s License required.
