**Once transferred out of trainee role and into your PCT role, this will be a 2 year work commitment.

Responsibilities:

Program Outcomes:

• Learn to form a relationship, communicate and interact one-to-one with residents
• Demonstrate sensitivity to residents’ emotional, social, and mental health needs
• Assist residents in attaining and maintaining functional independence
• Exhibit behavior in support and promotion of residents’ rights
• Demonstrate observational and documentation skills in the assessment of resident’s health,

physical condition and well-being

Certifications and Licensures:

Program Length: 75-hours, 5-Weeks

Days/Times: Mondays and Thursdays; 9:00 AM to 5:30 PM

Training Format:

• Classroom Discussion, Hands-on Lab Practice, and Clinical Application
• 19 Modules; State of Michigan Approved Curriculum
