Community Health Program Coordinator

Under the supervision of the Vice President Care Coordination, the Community Health Program Coordinator is responsible for overseeing the Community Health Program to include coordinating with Community Health Workers to provide outreach, health education, system navigation, client advocacy, and other services that reduce health disparities, improve health outcomes and increase health literacy of older adults living in underserved communities, including communities with predominately Spanish-speaking older adults.

  • Oversee and
coordinate the tasks of the Community Health Program including community
outreach, development and delivery of presentations, and progress reporting.
  • Provide
culturally appropriate health information and activities, in English and
Spanish, that increase health literacy, especially related to COVID-19, and
address social determinants of health and social isolation.
  • Working with
the Health Promotion Programs Coordinator, complete training in PAA-supported
Health Promotion programs and offer programs in Spanish.
  • Provide effective
and supportive supervision of program staff by mentoring, providing regular
feedback, and acknowledging efforts.
  • Enter and maintain client and program data
collected in accordance with PAA data entry standards.
  • Coordinate
with Virginia Department of Health (VDH) officials to ensure public health
initiatives, COVID-19 vaccinations, and other programs are offered in the
communities PAA’s serves. Attend staff meetings and other agency
activities as requested.
  • Perform other related tasks as required.
Essential Skills and Experience
  • Education:
Bachelor Degree in Public Health or related degree or combination of education
and experience equivalent to a Bachelor’s degree.
  • Bilingual
in English and Spanish.
  • Strong leadership, coaching, and training skills.
  • Ability to convey public health concepts and
community outreach skills to staff.
  • Ability
to effectively use computers, software, and mobile technology.
  • Ability to work collaboratively with other team
members.
  • Excellent interviewing, oral, and written

communication skills.

Special Requirements
  • Valid driver’s license, valid insurance, and use of personal
vehicle.
  • Proof

of full COVID-19 vaccination (as defined by the CDC) and annual flu shot required.

Full-time, M-F, 8:30 am – 5:00 pm, 40 hours/week, 1 day remote, Pay Range: $18.69 - $19.83.
