Description:
We're seeking a dedicated Social Worker / Clinical LCSW to continue the growth of our Social Worker Team at the Emory Clinic. As part of our team you will be responsible acknowledge and appropriately rpioritize referrals from interdisciplinary team members, patients, and families, with intial chart documentation entered within eight business hours of receipt of consult.You will attend and actively participate in Trials and interdisiplinary team meetings in order to screen for high risk patients, obtain and share information, identify appropriate lenghth of stay per DRG, advance the patient's plan of care, identify and take action to resolve barriers and to coordinate safe and appropriate discharge to proper level of care with the appropriate time frame.
Additionally, you will actively participate in Triad Data Meetings/ Team meetings, and collaborate with insuranace case managers and community care providers and initiate referrals to appropriate community resources.
MINIMUM QUALIFICATIONS:
- Masters in Social Work from an accredited Institution.
- At least 3 years of relevant social work experience required.
- Experience in an Acute Care or post-acute care setting desired but not required.
- Positions in this classification require LCSW licensure.
- ACM, CCM preferred.
