Specialty Case Manager - (Bilingual English/Spanish)

Position Summary:The Specialty Case Manager (Bilingual Spanish/English) will be a member of our Social Services department, working in support of patients in our Boricua Community Health Center. The Case Manager conducts assessments, identifies support services needed, provides internal/external referrals, advocacy, and social services for our patients.
Key Duties:-Conducts assessments and screenings to determine mental status, coping mechanisms, and specific support service needs.-
Provides case management assistance to patients including special populations. Case management assistance includes general and specialized screenings, assessments, advocacy, and referrals to internal and external services as needed.-
Obtains specifics resources that are required to improve patient health. Including but not limited to: Medical Durable Equipment, Home Care Applications, Medical Transportation, and other resources requested by provider.-
Provides resources for patients such as food pantries, nutrition information, housing resources, legal resources, DV programs, and health insurance assistance.-
Conducts intake and creates and updates patient charts. As part of intake, completes patient assessments to include screening for psycho-social and nutritional problems, and learning needs and provides appropriate referrals to case manager, LCSW, nutritionist, etc.
as needed. Assure that patients in crisis are immediately assessed by a provider or LCSW.-
Identifies patients in need of social work services or patients experiencing a crisis and refers patients to social worker or crisis responder. Provides support to crisis responders when working with patients on crisis.-Serves as an advocate and networking-professional to connect patients with services that are catered to their needs.-
Works in close collaboration with Licensed Social Worker to support patients that are experiencing crisis.
Minimum Qualifications:-Bachelor's Degree in Social Work, Psychology, Human Services, or related field of study.-
Bilingual Spanish/English, required.-
Team-player and willing to go the extra mile to support patients in our care.-
Proficiency with Microsoft Office.-
Ability to thrive in either an onsite or remote environment.-
Commitment to excellence and to providing quality service to patients.
Work Environment Type, Location, and Schedule:
Onsite, Monday to Friday 8:00 a.m. to 5:00 p.m.
Starting Compensation: Minimum: $21.1538/hour ($44,000 annually); Maximum: $21.1538/hour ($44,000 annually)
