Responsibilities:
The CHW works collaboratively as part of the ECM team and is responsible for supporting ECM patients for their complex care needs through education, outreach, and engagement primarily in a field-based capacity. Assist patients who have difficulty navigating the healthcare system due to cultural or linguistic barriers through supportive engagement activities, including but not limited to, accompaniment to appointments as needed, distribution of community resources and health promotional material, and patient advocacy.Work proactively both independent and with other ECM care team members to achieve better health outcomes.
Qualifications:
EDUCATION, EXPERIENCE, TRAINING- High school diploma or GED required
- Has successfully completed an approved Community Health Worker program or will complete within six (6) months of hire
- Two (2) years of experience as a Community Health Worker or two (2) years of experience working in community outreach, customer service, or within a medical office, or a Behavioral Health or Substance Use Disorder program required
- Knowledge of the community the CHW will be working in, especially non-professional resources, and their reputation in the community
- Understanding of and sensitivity to mental health conditions and addictive disorders
- Awareness of the impact of unmitigated bias and judgement on health; commitment to addressing both
- Understanding of, and a commitment to, high preforming team practices
A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $21.00 to $27.24. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
