Psychiatry and Behavioral Health at Greenwich Hospital is seeking to employ a full-time clinical social worker with specialties in treating a spectrum of psychiatric diagnoses, including mood, anxiety, post-traumatic stress, and substance-related disorders.The clinical social worker will provide evidence-based and solution-focused treatments to patients with these and other clinical conditions, collaborating with psychiatrists, nurse practitioners, psychologists and other providers in a multidisciplinary outpatient setting.

Responsibilities:
- 1. Interact professionally and with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, genders, sexual orientations, lifestyles, and physical abilities.
- 2. Complete comprehensive biopsychosocial assessments and/or psychiatric evaluations.
- 3. Develop and maintain a treatment plan responsive to patient/family concerns, problems and clinical impressions.
- 4. Use a patient and family centered approach to engage individuals and families in a plan of care that addresses their identified needs.
- 5. Provide education and counseling to individuals and families.
- 6. Utilize recognized best practices to provide individual, couples, family and/or group psychotherapy and/or crisis intervention as indicated.
- 7. Consult and collaborate with external providers/agencies/organizations including but not limited to protective services, court systems, outpatient therapists, work/education settings and housing supports.
- 8. Provide referrals to community-based services for individuals and families as indicated.
- 9. Provide information, consultation and professional support to interdisciplinary team members and colleagues within the hospital and community.
- 10. Facilitate and/or participate in interdisciplinary rounds and other patient related discussions to promote integrated care and improve patient outcomes.
- 11. Demonstrate initiative and leadership in developing services and programs to enhance patient and family care, including development of evidence-based practices.
- 12. Contribute to individual, team, departmental and organizational quality assurance and performance improvement.
- 13. Document timely, accurate and clinically relevant entries of social work activities in the electronic medical record.
- 14. Mentor/supervise and/or participate in projects/ work groups as assigned by supervisor.
- 15. Navigate systems and provide advocacy for patients and families as indicated.

- 16.1 Maintain and enhance knowledge base and skill set through educational opportunities and meet expectations set forth by State licensure, Hospital and departmental standards.
Qualifications:
EDUCATION
Master's Degree from an accredited school of social work.
EXPERIENCE
Individuals with experience treating a broad spectrum of psychiatric conditions and working in a collaborative, multidisciplinary clinical setting; training in cognitive-behavioral interventions; who embrace a biopsychosocial model of understanding and treating behavior and mental health; and are committed to program development are encouraged to apply.
LICENSURE
All candidates must have a license (LCSW) to practice as a clinical social worker.
SPECIAL SKILLS
The LCSW will possess the following special skills: empathy, active listening, critical thinking, strong organizational and computer skills, and the ability to effectively communicate with and advocate for patients and families.
