Physical Therapist Home Health

Shift: Day 5x8-Hour (08:00 - 16:30)

• Work location: Onsite; Santa Rosa, CA -

Compensation: $60 to $66 per hour

Duties and Responsibilities -

Conduct thorough evaluations of patients' physical abilities, mobility, strength, balance, and functional limitations.-

Identify any movement disorders or physical impairments that may affect patients' daily activities.-

Create individualized treatment plans based on patient evaluations, goals, and needs, ensuring alignment with physician orders.-

Establish realistic and measurable short-term and long-term goals for rehabilitation.-

Provide hands-on therapeutic interventions, including exercises, manual therapy, modalities (e.g., heat, cold, ultrasound), and gait training.-

Instruct patients on proper techniques for exercises and activities to promote recovery and prevent further injury.-

Teach patients and their families about the importance of physical therapy, self-care techniques, and home exercise programs.
