Licensed Vocational Nurse (LVN) Follow Up Coordinator

Overview:

This position can be filled in Dallas, Fort Worth, Waco, or Austin.

The Licensed Vocational Nurse (LVN) Follow Up Coordinator provides all functions required to monitor lab results and referrals, including, but not limited to positive sexually transmitted infections, abnormal labs, Gender Affirming Hormone Therapy (GAHT) follow-up, Pap, Colposcopy, and Loop Electrosurgical Excision Procedure (LEEP) results.
Contacts and informs patients of their follow-up plan and/or need for a referral and makes appointments for follow-up visits when indicated. Ensures the appropriate County Health Department is contacted when a positive result is received for a reportable infection.
Delivers high-quality reproductive health care and family planning services for all patients. Ensures productivity expectations, customer service and compliance standards are maintained. Abides by the organization’s mission in performing job duties.

Responsibilities:

  • Assumes overall administrative responsibility for functions relating to follow-up for assigned health centers after the care plan has been created by the Clinician.
  • Closely adheres to the Medical Standards and Guidelines (MS&G’s) when completing follow up.
  • Practices a patient focused approach to healthcare as demonstrated by interactions showing respect, knowledge, responsibility, compassion and access to care.
  • Provides education, accurate information and answers questions objectively about medical services offered at the affiliate with special focus on sexually transmitted infections in a caring, non-biased, non-judgmental and non-directive manner.
  • Explains medication and education materials to patients to ensure understanding of materials and services, adjusts as necessary and ensures that patients with limited English proficiency receive information in the language which they best understand.
  • Reviews test results with patients and when appropriate, educates patients with abnormal results regarding the nature of the problem, method and treatments prescribed, and instructions and information about proper follow-up, including referral to appropriate medical, financial, and social service resources when necessary.
  • Tracks and follows-up on abnormal results in the patient record in EHR system including accurately documenting all attempted contacts.
  • Generates letters for abnormal results including reviewing the printed letters, confirming the letter reflects accurate lab test result information, and ensuring the letters are mailed or submitted by the electronic health records (EHR) portal within required timeline
  • Investigates and reviews missing lab results by checking with the lab by phone or website and contacting health center staff when needed to reconcile lab orders.
  • Provides Occupational Health activities in conjunction with organization’s Occupational Health related-policies for employees including review and assessment of vaccine records and administration of Hepatitis B vaccine, flu vaccine and venipuncture for Tuberculosis (TB) testing as needed.
  • Monitors and tracks staff annual TB Assessment forms to ensure compliance.
  • Trains new clinicians in the PPGT follow-up process.
  • Provides compassionate care to patients using a holistic approach ensuring that all patients’ needs are met.
  • Mentor other Follow up Coordinators and health center team members as needed.
  • Monitors clinician approval queue in the electronic health records (EHR) system and advises Clinician when plans need to be created.
  • Assists with the follow-up of referrals.
  • Reviews system reports daily and determines which patients must be contacted.
  • Performs annual Occupational Health employee file audits in coordination with Human Resources and Risk and Quality Management.
  • Has unrestricted access to patient protected health information (PHI) on paper and electronic forms health records for purposes of treatment, payment, and/or healthcare operations. The use of a patient’s protected health information should be limited to information needed for the specific task that is being performed or requested by the individual patient. Disclosure of any patient information must be for purposes of treatment, payment or healthcare operation OR must be accompanied by a valid patient authorization. Must adhere to minimum necessary rule.

Qualifications:

High School Diploma or equivalent + 1 year of LVN experience. Reproductive health care experience preferred. Possess a valid licensed vocational nurse (LVN) issued by the Texas Board of Nursing.
