Responsibilities:

As an Occupational Therapist with Toomey, you’ll deliver comprehensive, high-quality care to clients in their home, following best practices and maintaining compliance with agency policies.

What you'll do:

  1. Provide initial and annual assessment of activities of daily living skills, occupational therapy screening and/or full assessments, as necessary, for individuals who are seen as in need of this type of assessment.
  2. Serve as a consultant to the QIDP, Program Coordinator, and other relevant personnel within the program evaluating current occupational therapy needs as they relate to improoving functioning levels, the otherall individual program plan for specific individuals and in the development of goal objectives for specific individuals, and the p[rovision of adaptive equiptment if necesary.
  3. Visit the residence on an as needed basis to establish relationships with and observe the individuals in our care, discuss progress and/or concerns with direct care staff, and meet with QIDP/Program Coordinator to revise and/or create individualized program plans.
  4. Provide periodic in service training, as necessary, to staff on occupationla therapy needs opr specific individuals, adaptive equiptment, possible goals, etc.
  5. Provide documentation of residents' occupational therapy status as deemed appropriate.
  6. Attend annual reviews as deemed necessary by the Program Coordinator.

Qualifications:

Qualifications
  • Must posses a Current Occupational Therapy licensure in New York and have a Minimum of 1-year as an Occupational Therapist.
  • Currently registered by New York State and the American Occupational Therapy Association.
  • Have the ability to effectively communicate with patients, families, and facility personnel.
  • Current New York State driver's license and automobile insurance.
