Job Summary
The Case Manager identifies patient biopsychosocial needs and connects patient with appropriate community resources. The Case Manager interacts directly with patient and guardians to participate in the discharge planning as per the Treatment Plan goals/interventions.They will also coordinate referrals and discharge appointments and collaborates with other service providers and community resources as appropriate.

Qualifications:
Required Qualifications- Bachelor’s degree in counseling psychology, psychology, or other closely related health degree
- 18 months experience providing substance use, mental health, or co-occurring treatment services in a behavioral health or related human services setting
- Ability to complete Crisis Prevention Intervention (CPI) training and successful completion during new hire orientation.
- BLS required from American Heart Association
