Part time, 48 Hours per pay period, Day shift

Position Summary:

Provides innovative, high-quality care as an integral member of our quality improvement team. Responsible for the promotion of rational, safe, and cost-effective drug therapy in collaboration with both internal and external primary care providers and specialists, nurse care managers, and other non-clinical roles.

Identifies and helps resolve medication therapy opportunities while co-designing individualized therapeutic regimens for optimal management of chronic disease states and preventative care. Interfaces virtually or telephonically with our members and their caregivers to deliver Medication Therapy Management (MTM) services and help drive better individual and population health outcomes while preventing avoidable ED visits and hospitalizations.

Responsibilities:

  • Provides virtual Medication Therapy Management (MTM) services and other pharmacist support services as needed.
  • Contributes in the development of a medication management action plan to achieve member self-management goals and clinical outcomes with measurable parameters and timeline for monitoring and follow-up. Facilitates patient understanding of the physician’s treatment plan, including but not limited to medication adherence, preventive care, and self-management skills.
  • Provides direct support to clinical team members for all general drug information, dosing, interaction/adverse effect avoidance questions, or member-specific needs.
  • Participates in staff education through presentation of medication management in-services, development of educational tools for clinical and non-clinical team members, and training/coaching/facilitating all roles on consistent maintenance of a member's best possible medication management care plan.
  • Facilitates the patient’s appropriate navigation of the healthcare system and supports patient understanding of personalized healthcare needs.
  • Acts as an advocate for an individual's healthcare needs by identifying and communicating potential needs to the provider and/or in-office clinical staff.
  • Understands the intricacies of patient medication assistance programs, as well as third party medication tier structure and formularies.
  • Supervises technicians and other support staff to ensure patient care needs are met.
  • Assists in the development of new projects.
  • Responsible for compliance with all state and federal pharmacy regulations and requirements.
  • Participates in community education programs to provide medication information.
  • Actively participates in quality improvement activities. Each employee will contribute to the continual evaluation of departmental performance as well as the implementation and measurement of improvement activities that increase the quality of care provided to patients.
  • Attends meetings as required and participates on committees as directed.
  • Precepts pharmacy students and/or pharmacy residents in specialty area.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications:

Minimum Education:

Licensed pharmacist in the state of Tennessee; completion of a PGY-1 Pharmacy Practice residency. PGY-2 Ambulatory Care Pharmacy residency or a minimum of two (2) years of experience as a clinical pharmacist in an ambulatory care setting preferred.

Minimum Experience:

One (2) to three (3) years of experience in a clinical setting. Must be PC proficient with working knowledge of Windows, Word, and Excel.

Current state pharmacy license
