Duties:

• Deliver social work services to patients and families in a high-paced ER environment.
• Assess psychosocial challenges and provide crisis intervention support.
• Coordinate patient discharge planning and follow-up care using ACM and Careport.
• Maintain accurate and timely records in EPIC.
• Collaborate with healthcare teams to address patient needs efficiently.

Skills Required:

• ER experience with expertise in EPIC and care management systems.
• Familiarity with ACM and Careport for discharge coordination.

Education:

• Master’s in Social Work (MSW) and an active LMSW license.
