Hospice Admissions RN
Overview:
The Admissions RN is responsible for all aspects related to the initial patient assessment and plan of care upon admission for patients across the life span (perinatal, pediatrics, and adults) according to established standards and practice.Responsible for providing information about services to potential patients, referral sources, and providing education to the community as needed. Responsible for skilled nursing assessment and hospice plan of care in order to maximize the comfort of patients and families consistent with The Elizabeth Hospice standards.
As a member of the Interdisciplinary Group, works collaboratively with the patient’s healthcare team to ensure continuity of care across care settings.

Our Mission: To enhance the quality of life for those nearing the end of life’s journey and for those who grieve.Salary: $45-$61 per hour. Compensation is determined by years of licensure.
Work Schedule: Full-Time, hours and days vary, 8-hour shifts beginning between 10am and 1pm. Candidate needs to be available 2 weekends per month.
Work Location: San Diego County and South Riverside County, CA
Travel: Travel is required for patient visits.
Responsibilities:
- Performs visits as needed to assess and evaluate patients’ medical conditions, gather historical data and assess needs, as well as:
- Assists with admissions and discharges from hospitals and/or facilities to the site of Hospice care.
- Performs Admissions at the site of Hospice Care.
- Other Hospice nursing visits as assigned.
- Advocates for patients and families, ensures their preferences for care are incorporated in the development of the hospice patient’s plan of care.
- Effectively manages time to meet department expectations in regards to admissions.
- Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care.
- Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.
- Assess and evaluate every patient to determine the appropriate plan of care (POC).
- Obtain relevant information and supporting documentation.
- Independently assess patient/family immediate care/symptom management needs in a timely manner. Assess patient/family need for immediate psychosocial, emotional, bereavement and volunteer support in compliance with regulations.
- Consult with patient’s chosen attending MD, the Hospice Medical Director/Physician designee and other medical personnel.
- Develop the Hospice patients’ Plan of Care and communicate initial assessment, admission information, financial information, plan of care, and changes to the plan of care to the IDG on all assigned cases.
- Provides appropriate follow-up to outside case managers in regards to insurance and authorizations.
- Reviews borderline/complex cases per protocol. Adheres to the Non-Admit Policy
- Coordinate admission to hospice with other facilities, assisting with implementation and coordination of services.
- Effectively manages symptoms.
- Participates in training and mentoring of new staff.
- Keeps current with all department policies and procedures.
- Adhering to the Elizabeth-way standards is a requirement in performing all job duties.
- Collaborates with other staff members across teams and departments to assist in the resolution of issues and complaints that may arise during the admission process.
- Establishes, builds and nurtures relationships with identified sources in the community including hospitals, facilities and other referral sources. Participates in education and marketing activities in the community.
- Participates in quality assurance and performance improvement activites to continuualy improve the delivery of hospice care services and ensure compliance with established standards.
- Adheres to all related regulatory requirements and protocols.
- Other job duties as assigned.
Qualifications:
- Must have an Associate’s Degree, Bachelor’s Degree preferred, and at least 12 months experience as a Hospice Nurse preferred, or equivalent combination of education and experience.
- Current CA RN license.
- Current CPR certification and negative TB skin test
- Prefer certification as hospice nurse.
- Computer skills (word processing)
- Current California driver’s license and proof of auto insurance required.
- Bilingual (Spanish/English) preferred.
