Home Health Physical Therapist

You will conduct thorough evaluations, set realistic goals, and collaborate with other healthcare professionals to ensure comprehensive patient care. At Expert Healthcare, we prioritize a supportive environment where both our staff and patients thrive.

Schedule: Monday - Friday, 40 hours per week

• Perform evaluations and develop tailored treatment plans based on patient needs.
• Implement therapeutic exercises and interventions to facilitate recovery.
• Monitor patient progress and modify treatment plans as necessary.
• Educate patients and their families on rehabilitation exercises and equipment usage.
• Coordinate with other healthcare providers to ensure optimal patient care.
• Document patient care and progress in compliance with regulatory standards.

• Current and valid Physical Therapist license in the state of practice.
• Degree in Physical Therapy from an accredited program.
• Home Health Experience Preferred.
• HomeCare HomeBase Charting Experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a caseload.
• Basic life support (BLS) certification is required.
