Case Manager LPN

Type: Five - month contract with an opportunity for extension or full-time offerSchedule: 8AM – 5PM Monday – Friday
Pay: $30 - $34.50 per hour based on experience
Responsibilities of the Case Manager LPN:
- Helping manage our high risk population.
- Triage phone calls
- Case management
- Hospital tracking
- Disease management
- Wound care
- Patient education.
Qualifications for the Case Manager LPN:
- High School Diploma or GED
- Active LPN license in Florida
- Weekly Pay
- 401(k) Plan
- Skills Training
- Excellent medical, dental, and vision benefits
Available paid leave may include Paid Sick Leave, where required by law; any other paid leave required by Federal, State, or local law; and Holiday pay upon meeting eligibility criteria.

Pay Details: $30.00 to $34.50 per hour
