Staff Guardian
Overview:
Community First Solutions, Lifespan team is looking for a full-time, Staff Guardian to join our team. This employee will be the Legal Court-appointed Guardian and advocate for clients who have been deemed incompetent by the local probate courts.Legal court responsibilities include the fiduciary acceptance (a legal entity that has the responsibility and power to act on behalf of another person in making decisions in the best interest and providing suitable maintenance when necessary).
The Staff Guardian works closely with the team as they use their skillsets to work with an often-difficult population who are incompetent in making independent decisions while respecting their choices as much as possible. This position needs to use their experience to seek to understand all the options before making decisions regarding the client’s medical consents, living arrangements, coordination of benefits.
After one year in this role, the Staff Guardian 1 is eligible to become a National Certified Guardian with the opportunity for advancement. This position requires candidates to conduct clients home/or facility visits for clients in the Butler/Warren County area.The position is primarily working remotely, though the candidate will be required to regularly come into the office. Initial training will be conducted on-site.
Shifts Available –Day Shift: Monday- Friday
Telephonic On-Call Rotation with team 24/7 to provide consent to treat for medical needs of clients
Pay: Starting at $18 to $22 per hour depending experiance

Responsibilities:
Staff Guardians assist in making decisions and following through on decisions made for clients such as living arrangements, medical decisions, quality of life improvements, etc. In this role, assistance will be provided by Staff Guardian II and III team members.
- Manage and maintain a caseload including all required documentation in accordance with agency policy and procedure.
- Make required visits to clients within their home setting (i.e., nursing home, group home, independent living, assisted living, apartment, house)
- Attend interdisciplinary care conferences
- Complete assessments for clients referred to the program
- Participate in court and legal processes pertaining to clients
- Maintain responsibility for clients' finances and applications for benefits as appropriate (i.e. work in coordination with Guardian of Estate, payee, trust officers, etc.)
Remote Requirements: Must have high-speed internet
Must have an area of your home free of noise and distracts that can safely harbor clients information.
Qualifications:
Minimum Associate degree or acceptable certification such as certified case manager, LPN, Licensed mental health counselor. LSW, Nurse, or Bachelor’s degree from an accredited institution in the related field working with diverse clients including elderly and those with mental health and intellectual disabilities diagnosis preferred.One-year social service experience working with individuals with mental health issues and/or developmental disabilities. With experience working with a variety of systems such as the courts, mental health, DD, medical, aging, protective services, and long-term care is preferred.
Guardianship certification must be obtained within 18 months of eligibility.Must be an Ohio resident.
Valid driver’s license and proof of auto insurance.
