The FACT Case Manager provides outreach and referrals to parents and families challenged with the effects of substance abuse. The Case Manager links the program participant to appropriate substance abuse intervention including detoxification, rehabilitation, and intensive outpatient treatment.

The case manager provides coordination of services between participants, family, and multiple providers and educates and counsels participants and families on issues related to substance abuse, including achieving and maintaining sobriety and relapse prevention.

Candidate qualifications:

+ Bachelor’s degree in social work or another human service field required.

+ Certified Alcohol and Other Drug Counselor (CADC) preferred.

+ Experience working with families experiencing alcohol and substance abuse required.

+ Valid driver’s license, insurance, and access to a reliable vehicle required.

Job details:

+ Compensation: Range starts at $24.20 per hour, offers are commensurate with experience.

+ Benefits: Medical/dental/vision insurance, 3+ weeks paid time off in the first year including the week between Christmas and New Year’s Day (PTO increases with tenure); 11 paid holidays, supplemental insurance options, 401(k) with match (more benefits details here).

+ Location: Hybrid (remote/in office), primarily visits with families in Kane, DuPage and McHenry counties.

+ Schedule: Full-time, hourly; regular business hours with remote flexibility. Occasional evening hours required for program activities. Frequent local travel required (up to 60% of time).
