Advance Practice Provider (APP), Physician Assistant or APRN
Department: YNHH Hospitalist Service
The primary function of Advance Practice Provider (APP) working for the Hospitalist Service is to provide comprehensive care to medicine inpatients in collaboration with attending physicians and under the direction of the APP Manager(s) and Director of the Hospitalist Service.Each APP has a designated supervising/collaborating physician.

Responsibilities:
Clinical Practice:
- At the instruction of the APP Manager and Director or his/her designee, assists in the care of medicine inpatients assigned to the Hospitalist Service, ensuring safe, high quality, patient-centered and efficient care.
- Collaborates with attending physicians (Hospitalist Service or Private) to execute patient care. Holds primary responsibility for medical record documentation), ordering tests/procedures and communication with consultants.
- Identifies self in EPIC as covering provider by 8:30 am, or earlier if patient assignments are available. Provides the patient individualized business and/or picture cards and writes name daily on the patient’s white board to identify the practitioner taking care of them on any given day.
- Rounds daily on assigned patients and regularly communicates patient status, results and changes in the plan of care to the attending physician.
- Ensures all patient handoff sections are updated in EPIC for assigned patients by 6:00 p.m.
- Serves as a liaison and interacts with the rehabilitation staff, Food and Nutrition staff, and other ancillary services to ensure timely and comprehensive care for all patients. Attends transition of care rounds daily.
- Serves as a liaison to patients and their families.
- Educates patients and families about transition needs. Completes discharge paperwork, including the W-10 and prescriptions (but not the discharge summary). Reviews discharge instructions with patients and nursing at time of discharge. Coordinates outpatient follow-up.
- Performs necessary invasive diagnostic and therapeutic procedures, as credentialed by Yale-New Haven Hospital Office of Physician Services or under the supervision of an Attending physician
Qualifications:
EDUCATION: Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
EXPERIENCE: Prefer clinical working experience as a NP/PA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.
LICENSURE: Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License, when indicated for pediatric position profile PALS, when indicated for neonatal care NRP Certification.Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty. OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile ACLS/BLS Certified, when indicated for position profile DEA License, when indicated for pediatric position profile PALS, when indicated for neonatal care NRP Certification.
Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty.
Additional Information:
- Identifies informational needs and maximizes opportunities in the clinical setting for informal education of team members, patients and families.
- Acts as a role model/preceptor for PA students. Provides related teaching and education to PA students from the Yale University School of Medicine Physician Associate Program.
- Assists in orientation to new APPs working for the Hospitalist Service.
- Participates in Hospitalist Service educational conferences.
- Participates in a level of continuing medical education (CME) activity annually that improves the quality of care provided by the Service.
