This position is responsible for providing administrative support to the care management programs at the direction of the management team.
EDUCATION: Associate’s or Bachelor’s Degree in Business Administration Preferred.
EXPERIENCE/ ABILITIES:- Minimum five (5) years working in a secretarial/administrative support role in a health care setting.
- Computer experience with creating word processing documents, spreadsheets, and presentations.
- Able to solve problems and work with minimal supervision.
- Able to perform basic accurate mathematical skills.
- Able to provide excellent customer service when answering phones and assisting callers.
- Requires outstanding written and verbal skills.
- Requires outstanding people and communication skills.
- Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including members and their families, nursing facility staff, physicians, the health plan and any other stakeholders in the community.
- Provide administrative support to staff for care management programs.
- Assist Care Management staff with outreach as appropriate.
- Coordinate and attend CSNP IDT – prepare list, coordinate tracker, and document deliverables and team recommendations for weekly IDT.
- May assist with notifying team of transitions of care that occur (both admissions and discharge).
- Distribute a list of members who have had their initial or annual HRA completed (for review during IDT meeting) and assure that an ICT note is entered after the meeting.
- Strong administrative support to care management team; assists with obtaining information and distribution of requested materials such as spreadsheets and/or member lists.
- Complete level and case owner changes as directed by management.
- Schedule required CSNP meetings utilizing ZOOM/Teams as requested, assure that appropriate documents are available for sharing during meeting, and maintain record of meeting attendance.
- May assist with distribution of documents and routine correspondence when requested as back up.
- Assist with coordination of RPM, including sending notifications to the team regarding alerts.
- Provide cross-coverage for other administrative staff within the organization i.e. Administrative Assistant, etc.
- Receive incoming HIPAA/HCP/HealtheLink forms and distribute to the team as indicated.
- Receive PCP and Specialist ICP reviews-attach in EMR and notify team members as indicated.
- Send Transitions Packets to receiving setting per Family Choice guidelines.
- Assists RN with requests for levels of service by compiling data, entering authorizations, and faxing authorizations to participating providers, vendors and nursing/adult home care facilities after determination by RN or Medical Director as requested.
- Assist with data entry with UM Transitions of Care department.
- May assist with receiving and processing requests for non-emergent transportation by scheduling trips with approved vendor and faxing information to nursing/adult home care facilities.
- Send notifications to Transitions Group regarding CSNP daily transitions.
- Other duties as assigned.
- $20.50 - $21.00 per hour
