Overview:

Responsible for the overall clinical direction of the Assisted Living Program. As the clinical director, establishes, implements, and evaluates goals and objectives for Assisted Living Program that meets and promotes the standards of quality and contributes to the total organization and philosophy.

Will assess, plan, implement, and evaluate effectiveness of the nursing care delivery and communicate information appropriately to physician and the interdisciplinary team. Ensure adherence to organizational policies and procedures and all regulatory guidelines for clinical staff and ALP Program.

Responsibilities:

CLINICAL COMPLIANCE:

  • Maintains records according to Centers for Medicaid Services (CMS) and the New York State Department of Health (NYSDOH) Standards for Adult Care Facilities/Assisted Living Program (ALP).
  • Maintains current knowledge and adheres to all regulatory agencies’ procedures.
  • Maintains appropriate quality documentation of all resident services provided, according to program policy and procedures and regulations.
  • Maintains client confidentiality and adherence to HIPAA requirements at all times.
  • Completes all Care Plans, UAS NY, and any regulatory required documentation in a timely manner.
  • Conducts appropriate nursing assessment as required to facilitate admissions and care planning, and regulatory process requirements as USA NY, and other related processes for residents.
  • Demonstrates working knowledge of CMS, NYSDOH and regulatory agencies mandates/standards.

DOCUMENTATION:

  • Maintains records according to CMS and NYSDOH Standards for Adult Care Facilities/Assisted Living Program policies and procedures.
  • Participates in the development and annual review or revision of policies and procedures.

EMPLOYEE DEVELOPMENT AND LEADERSHIP:

  • Utilizes management and administrative skills to accomplish ALP facility goals.
  • Fosters problem solving abilities to staff.
  • Provides orientation, in-service education, approval of annual performance evaluations to all staff under supervision.

SUPERVISION:

  • Shares current knowledge and up to date clinical expertise with staff through formal and informal education.
  • Provides for the on-going evaluation and achievement of RN, LPN, and PCA clinical competencies.
  • Coordinates skilled nursing care and prescribed treatment to the residents in ALP per mandates and regulations.
  • Actively participate as part of interdisciplinary team with suggestions for interventions and recommendations for revised treatment plans to address resident concerns and issues.
  • Provides for the on-going evaluation and achievement of RN Manager clinical competencies.
  • Maintain communication with other team members to discuss progress and coordinates resident care efforts.
  • Coordinate skilled nursing care and prescribed treatment to the residents in ALP per mandates and regulations.
  • Provides clinical consultation through teaching, supervision, conferencing, and review of documentation.

QUALITY ASSURANCE AND PERFORMANCE IMPROVEMENT (QAPI):

  • Assist with the development, implementation and evaluation of Quality Assurance and Performance Improvement.
  • Analyze QAPI indicators and initiate remedial action as required. Maintain QAPIs threshold of errors below target level.
  • Effectively support QAPI initiatives through data gathering, analysis, and by actively participating in QAPI meetings.
  • Assists in resolution strategies and implementation of plans to address issues/problems with residents and staff as they arise.
  • Monitors delivery of resident/client care to assure quality expectations are met. Maintain an effective resident complaint/issue resolution system with ALP Administrator for areas of responsibility.

RESIDENT/CLIENT CARE:

  • Conduct periodic nursing assessments and evaluations for each resident and routine nursing care for ongoing or episodic illness according to the plan developed by the team.
  • Counsel resident families regarding measures to maximize independence as a solution to physical, emotional and environmental health problems.
  • Makes referrals to other services when resident conditions or situations requiring the service of other professional disciplines or products/services of outside companies or agencies.
  • Participates in the development and revision of the resident plan of care as a member of the interdisciplinary team.
  • Completes and maintain accident and incident reports.
  • Evaluates and approves new admissions and readmissions according to pre-admission standards, and obtains necessary clinical documentation.
  • Provides instruction and demonstrates nursing techniques to resident and care-givers.
  • Actively participates as part of the interdisciplinary team with suggestions for interventions and recommendations for revised treatment plans to address resident concerns and issues.
  • Initiates and develops upon admission, resident individualized care plans and review every 6 months, during significant changes and annually.
  • Maintains accurate, complete, and timely UASNY submissions.
  • Obtains timely medical and mental health evaluations from providers, case management evaluation, care plans (initially, quarterly, annually, or during significant change).
  • Maintains communication with other team members to discuss progress and coordinates resident care efforts.
  • Performs other duties as directed.

Qualifications:

Education:

  • Bachelor’s degree in nursing Required.
  • Master’s degree in nursing or other health related field or working toward completion, preferred.

Experience:

  • Minimum seven (7) years experience as a Registered Nurse, required.
  • Minimum five (5) years of progressively responsible clinical management experience in a health care setting preferred.

Knowledge, Skills and Ability:

  • Maintain clinical expertise and certification in an appropriate specialty from a nationally recognized nursing organization.
  • Maintain affiliations with an appropriate professional organization.
  • Ability and/or expertise to fully utilize computer programs and systems, equipment and other healthcare and systems relevant to this position.

Licenses, Certifications:

  • Current New York State licensed Registered Nurse, required.
  • Basic Cardiac Life Support (BCLS) certified.

Physical Requirements:

  • Consistent with that necessary to work in a variety of office settings.
  • Position requires prolonged periods of standing, reaching, and walking throughout the workday.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds for routine performance of essential functions.
